   • अपॉइंटमेंट विवरण

2. जानकारी का उपयोग
   एकत्र किया गया डाटा केवल निम्न उद्देश्यों के लिए उपयोग किया जाएगा:
   • अध्ययन फॉलो-अप
   • रिमाइंडर और संचार
   • मौखिक स्वच्छता तथा क्लिनिकल पैरामीटर की निगरानी

3. गोपनीयता
   सारी जानकारी गोपनीय रखी जाएगी। केवल अधिकृत अध्ययन कर्मचारी या नियामक अधिकारी (यदि आवश्यक हो) इस जानकारी को देख सकते हैं।
4. डाटा संग्रहण
   जानकारी इलेक्ट्रॉनिक रूप में अध्ययन अवधि तथा संस्थागत / नियामक नियमों के अनुसार संरक्षित की जा सकती है।
5. कोई व्यावसायिक उपयोग नहीं
   आपकी जानकारी किसी भी व्यावसायिक उद्देश्य के लिए न बेची जाएगी और न ही साझा की जाएगी।
6. बंद करने का अधिकार
   आप किसी भी समय एप का उपयोग बंद कर सकते हैं। इससे आपके अधिकार या चिकित्सा देखभाल पर कोई प्रभाव नहीं पड़ेगा।
